Apply for the Disability Tax Credit

To apply for the Disability Tax Credit (DTC), you must submit a T2201 form to the Canadian government. Use this checklist to prepare the form.

Complete Part A 

  • Complete Part A (Personal Information & Authorization) of the T2201 form. Be sure to fill out all required fields, including:
    • Full name
    • Date of birth
    • Mailing address and telephone number
    • Social Insurance Number (SIN)
  • If a supporting family member is claiming the credit, include their personal information, SIN, and relationship to you.
  • If transferring the tax credit, clearly state who will claim it.
  • If a representative will communicate with the CRA on your behalf, sign and date the Authorization section.
  • If applicable, indicate whether you want previous tax returns adjusted for up to 10 years.
  • Sign and date Part A of the form.

Review Your Eligibility for the DTC

  • Review the eligibility criteria for the DTC. To be eligible, you must have a severe and prolonged impairment that restricts your daily activities and/or requires life-sustaining therapy.
  • Prepare supporting documentation:
    • Gather any prior medical reports that explain your condition.
    • Use the Restrictions Questionnaire Tool, if desired, to summarize restrictions.

Ask a Medical Practitioner to Complete Part B

  • Choose a qualified medical practitioner (doctor, nurse practitioner, or specialist). Ask them to complete Part B of the T2201 form.
  • Ensure the medical practitioner clearly describes your restrictions and any life-sustaining therapy that is required.
  • Confirm that the form includes detailed examples of how your impairment affects your daily life.

Review the Completed Application

  • Verify that Part A and Part B are fully completed.
  • Check for missing signatures or unanswered sections.
  • Make sure the medical practitioner’s section is completed, signed, and dated.

Submit the Application 

Use one of the following methods to submit the T2201 form:

  • Submit online through CRA’s My Account.
  • Mail the application to the appropriate Tax Centre (listed on the form).
  • Call CRA to complete Part A and obtain a reference number for Part B.

Track Your Application

  • Keep a copy of the form for your records.
  • Monitor your CRA account for status updates.
  • Respond promptly if CRA requests additional information.
  • If your application is denied, consider reapplying with additional supporting details.

Communicate Effectively With Support Personnel

Use these tips to effectively communicate with support personnel. 

Follow up with your medical practitioner to confirm that Part B was submitted correctly and completely.

Clearly explain your situation and what type of help you need. If possible, prepare a short summary of how your impairment impacts your daily life.

Be specific when asking questions about the T2201 application for the Disability Tax Credit (DTC). For example, instead of “Am I eligible?” ask, “Does my impairment qualify under the CRA’s definition of marked restriction?”

If your medical practitioner is unfamiliar with the T2201 form, provide them with CRA guidelines or refer them to resources that explain how to complete Part B.

Ensure your support personnel understand that eligibility is based on functional restrictions rather than just a medical diagnosis.

If your application is denied, ask support personnel to help you identify missing details or unclear descriptions that may have led to the decision.
